The Chicago Southeast Diabetes Community Action Coalition (CSDCAC) needs your help in preventing and controlling diabetes in our community.  Diabetes disproportionately affects persons living on the Southeast side of Chicago.

CSDCAC will facilitate a FREE diabetes education program at your facility.  We ask that you provide the space and/or participants including diabetics, those at risk of diabetes and interested community residents.

The program consists of 10 workshops, each approximately 1½ hours. 
Sessions include:   FREE diabetes products
                             FREE educational materials
                             FREE expert consultations (podiatrist, dietician, pharmaceutical      
                             representatives, Medicare/Medicaid representatives, etc.)
Upon completion of the workshop, participants will receive certificates and a chance to win free diabetes supplies and/or medication.
